Level 4 Activities:

– Any activity involving distribution of items ON-SITE at MVBT to others outside the group

– Any activity involving distribution of items to others outside of the group OFF-SITE with personal contact

– Any activity involving unvaccinated adults or children INDOORS on-site at MVBT

– Any activity with >30 attendees

– Any activity requested by a Non-MVBT Organization ON-SITE at MVBT

Level 4 Activities DO require review by MVBT Health and Safety Committee and approval by the Temple Board.

Proposals should be submitted at least 2 months prior to proposed activity to allow time for review

Organizers must also contact the Building Activities Committee to reserve the facilities if activity is approved:

General Requirements for Level 4 Activities

The following MUST be strictly adhered to by any attendees of the activity:

A Level 4 Activity Proposal Form to document plans for the activity must be submitted. The H&S committee will review the proposed plan and make any suggestions to minimize exposure risks
A designated Health Officer must be identified for each activity
The activity must be OUTDOORS unless otherwise approved by H&S and Facilities to use indoor locations
All attendees must complete a Health Attestation Form (Online or Paper) on the day of activity
The Health Officer is responsible for:
- Confirming all attendees have completed Health Attestation form on the day of the activity (proof of completion must be reviewed)
- Collecting any PAPER Attestation Forms and entering them electronically into Jotform for record keeping
- Assuring all safety protocols are followed
If any attendees test positive for COVID19 within 7 days of the event/meeting/service, that attendee will notify the designated Health Officer or contact MVBT at (650) 964-9426
More than one Level 3 or 4 Activity may occur at the same time provided they are in separate buildings/areas and the different groups do not intermingle
There may be further requirements made by the H&S committee after reviewing the activity proposal

General Safety Protocols for Level 4 Activities

The MVBT Health and Safety Committee recommends the following as best practice measures to prevent to spread of COVID19 and protect the health of all attendees:

COVID19 Vaccination for those eligible is encouraged
Face masks worn at all times regardless of vaccination status
Gloves worn while handling items if event is for packaging or distributing items
Attendees who DO NOT LIVE in the SAME HOUSE should:
- Stay at least 6 feet apart and practice good hand hygiene
- Avoid holding or shaking hands, hugging, or kissing
- Limit sharing items
Eating/drinking together is discouraged. However, if individual food or drinks are consumed, attendees should be socially distanced (minimum 6 feet) from others, time spent eating/drinking should be minimized, and masks should be put back on as soon as finished eating. Hands should be sanitized after eating/drinking.
Buffet style food and sharing of food between attendees from different households is discouraged
Smaller groups activities are encouraged to minimize the risk of exposure
Use only the designated restroom facilities and only one person at a time.
There may be further more specific recommendations made by the H&S committee after reviewing the activity proposal

Fever ≥ 100 degrees Fahrenheit
New Cough
New Shortness of Breath or Difficulty Breathing
Sore Throat
Runny nose or nasal congestion
Headache

Nausea or vomiting
Diarrhea
Severe Fatigue or Muscle Aches
Chills or repeated shaking
New loss of taste or smell
